Please complete these forms to update information for Minnie Hamilton’s School Based Health Program
Mandi Johnson and Brandi Grogg are at Arnoldsburg Elementary School
Tuesday and Thursday mornings throughout the school year.
They can complete well child checks, immunizations, check for illness like strep or covid. School Based Health saves students from missing classes to be transported to and from doctor's appointments.
If you need an extra form, please call AES and ask Jackie to send a form with your child.
Let's keep the kids happy and healthy this year. With this program available sometimes an out of school appointment isn't necessary when we have them right here in the school!

from Group Sessions to Private Packages for children or adults.
We’ve also started a Youth Stroke Club!
Lessons and Stroke Club last a half-hour and are designed to
improve performance of swim skills, in addition to simply
increasing comfort and endurance in the water.
At the end of each Session, participants are given a Checklist
showing what the student learned and what skills need to be
mastered before moving to the next Level.
The Stroke Club receives a Coach’s Assessment.
All swimmers review safety in and around water
